Deadline and Procedures for Sports Physicals

The due date for fall sports physicals is Monday 8/31 at 3:30 PM. Please be aware there will be no exceptions made for late forms. The physical review process will take place three times per school year prior to each season. It is important that all forms are complete as incomplete forms could delay or prevent student participation.

Please note that initial exam must have taken place on or after 9/10/14 to be eligible for the fall season. Tryouts for boys and girls soccer, girls field hockey and practices for cross country begin on Wednesday, September 9th.

Please contact Mrs Latini, School Nurse, with any questions at latinie@tabschools.org or 268-0153 x1005. You may also contact the main office at Olson School. Forms are available in the main office or on the school website at http://www.tabschools.org/Page/115
